St Andrews Church Hall Booking Form

Please ensure you have fully read the Church Hall Terms and Conditions of Hire, and consulted the Church Hall Calendar for availability as shown on the Church Hall page of this website before submitting your booking form to the booking administrator.

All questions refer to the HIRER ONLY and all information given should therefore be that of the hirer alone. The hirer must be over the age of 18 and agree to hold the full
responsibility for the hiring period and the event taking place during that period.

What is the reason for hire? (family celebration, fundraiser, party, etc) If a children’s party, please give the age of the child involved.

If the answer is yes, please be sure to have full details and a copy of their public liability. If the Church Hall is damaged in any way and it is the third-party company’s fault, YOU as the hirer will be held accountable for the damage caused and any costs incurred will be sent to you.

The fee is £12 per hour/£6 per half hour and to secure a booking a £30 returnable deposit must accompany this form.

Payments can be paid either by Bank transfer or by Cheque (please ask administrator if address details required).

Cash payments are not accepted.

Cheques should be made out to MEDSTEAD PCC For Bank transfers details as follows:

Natwest Bank Account number: 83126104 Sort Code: 60-01-13 Reference: Deposit followed by your name or initials.

Hirers are invoiced separately for hire charges once booking has been confirmed.

Declaration:

I, as the Hirer, acknowledge that I have read and agree to comply with the St Andrew’s Church Hall Booking Terms and Conditions and I also agree to accept full responsibility for this booking. If appropriate I also confirm that I will maintain the relevant Public Liability Insurance and that I will cover damage to the church’s property and injury to members/ users. I agree I will provide a copy of my licence certificates etc to the booking secretary if asked.
